Supporting documents

  1. 13-digit barcoded ID or Smart ID (applicant and spouse)
  2. Medical or functional assessment report
  3. Clinical information or referral form
  4. Referral form if previously rejected on medical grounds

How to register and apply

  • Apply at nearest SASSA office or online where available.
  • Authorized person may apply if applicant is too old or sick.
  • Keep receipt/reference number as proof.
  • No fee required to apply.
